COVID-19 Vaccination Verification - Fall 2021
 
 
On August 11, 2021, the California Department of Public Health (CDPH) issued a State Public Health Order (Order) for all public and private TK - 12 schools to verify vaccination status for all workers by October 15, 2021. Employees who are not fully vaccinated must be tested at least once a week.
Per the CDPH Order:
 
I. All schools must verify the COVID-19 vaccine status of all workers.
 
Pursuant to the CDPH Guidance for Vaccine Records Guidelines & Standards, only the following modes may be used as proof of vaccination:
 
a. COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided and date last dose administered); OR
b. a photo of a Vaccination Record Card as a separate document; OR
c. a photo of the client's Vaccination Record Card stored on a phone or electronic device; OR
d. documentation of COVID-19 vaccination from a health care provider; OR
e. digital record that includes a QR code that when scanned by a SMART Health Card reader displays to the reader client name, date of birth, vaccine dates and vaccine type; OR
f. documentation of vaccination from other contracted employers who follow these vaccination records guidelines and standards.
 
In the absence of knowledge to the contrary, a school may accept the documentation presented as valid.

II. Testing requirements
 
Workers who are not fully vaccinated, or for whom vaccine status is unknown or documentation is not provided, must be considered unvaccinated and must be tested at least once a week. The following defines workers who are not fully vaccinated and must be tested weekly:
 
*Asymptomatic unvaccinated (a person who has not received any doses of COVID-19 vaccine or whose status is unknown).
*Incompletely vaccinated (a person who has received at least one dose of the COVID-19 vaccine but do not meet the definition of fully vaccinated).
 
Unvaccinated or incompletely vaccinated workers must also observe all other infection control requirements, and are not exempted from the testing requirement even if they have a medical contraindication (a condition that makes the vaccination not advisable) to vaccination, since they are still potentially able to spread the illness.  Previous history of COVID-19 from which the individual recovered more than 90 days earlier, or a previous positive antibody test for COVID-19, do not waive this requirement for testing.
 
Schools with workers required to undergo workplace diagnostic screening testing should have a plan in place for tracking test results and conducting workplace contact tracing, and must report results to local public health departments. We are in the process of putting this plan into place and will communicate further details as they are finalized with the companies who will conduct the testing.

1. You may upload your verification of COVID-19 vaccination in "pdf", "jpg", or "png" formats in this Google Form (https://bit.ly/3BbIChB).
 
OR
 
2. You may make an appointment with the Human Resource Department to drop off a copy of your verification. Please call (562) 926-5566, ext. 21174 to schedule an appointment.
(If you will be dropping off your letter at the District Office, only one person will be allowed in the HR Office at a time and physical distance will be maintained while waiting. And, please don't forget to wear a mask.)
 
 
 
 
LA County Walk-In Vaccinations through Monday, April 26, 2021.
 
A limited number of walk-in (no-appointment-needed) registration spots are being offered at all County-run mass vaccination sites through Monday 4/26 while supply lasts.
 
Anyone 16 and older living or working in Los Angeles County can now get vaccinated without booking an appointment at the locations provided below (minors must be accompanied by a parent or guardian).

COVID-19 Vaccination at Cerritos High School this Wednesday!

The vaccination clinic at Cerritos High School is set for Wednesday, March 10, 2021 (first dose) & Wednesday, April 7, 2021 (second dose), from 11:00 AM to 3:00 PM.  The vaccine provided by the county for this clinic is Moderna which requires 28 days between the first and second doses.
