California Healthy Kids Survey for 6th, 7th, 9th, & 11th Grade Students

In March 2026, students in grades 6, 7, 9, and 11 will take the California Helthy Kids Survey (CHKS). This anonymous survey helps us understand how students feel about their school, their health, and their safety. We use this information to make our school a more supportive place to learn. Parent/Guardian will receive information prior to its administration.

This anonymous survey gives our students a voice. By sharing how they feel about school safety, their health, and their connections with others, they help our district understand how to better support every student's well-being and success

The CHKS is a key tool we use to improve our schools. By looking at the results, we can make better decisions about how to support student learning and well-being. This process also ensures our school stays on track with our district goals outlined in our Local Control and Accountability Plan (LCAP).

PARENT CONSENT:

  • Sixth-grade students need “active” parent consent: Parent or Guardian must opt in by submitting their signed permission.
  • Seventh, Ninth, and Eleventh-grade students need “passive” parent consent: Students may participate unless parents/guardians notify the school they wish to opt out.

The CHKS Elementary Core Survey assesses student attitudes, behaviors, and experiences related to school, academic and social-emotional learning, positive development, overall health, and well-being. It contains many of the same or similar items as the middle school version, so that cross-survey comparisons can be made. The wording is simpler and developmentally appropriate. The CHKS Secondary Core Module (Middle and High School) assesses school climate and safety, pupil engagement, student supports, bullying, and substance abuse.  Demographic questions help identify the needs of key populations, including racial/ethnic groups, foster youth, economically disadvantaged, and English language learners. The CHKS Behavioral Health Module (Middle and High School Only) is a multiform that assesses adult and peer supports, mental health supports and access to services, social isolation, body image, responses to trauma, stress-associated health conditions, emotion regulation, self-harm behavior, and precursors to and consequences of substance use.  Students are randomly given either Form A or Form B during administration so that no student answers both sets of questions.
