Reasonable Assurance Letter?

If you recently received a NOTIFICATION OF REASONABLE ASSURANCE from ABC Unified School District.  We are providing a few different ways for you to return your signed Reasonable Assurance letters to the district.  If you also received a Verification of Immunizations (Influenza) letter/exemption form, you may also use one of the following methods to submit your exemption form.

Fax your letter to (562) 404-9349.
Mail your letter to:

    ATTN: Reasonable Assurance Letter

Upload your signed letter/form in a Google Form (save your letter as a "pdf", "jpg", or "png" format).

Drop off your letter at the District Office Lobby Monday through Friday from 8 AM to 4:30 PM.
